This case illustrates it’s not a wound care failure but a collaboration failure. The importance that healthcare professionals, and not only nurses, should be aware of pressure ulcer risk factors, the consequences that may occur when you don’t address the risks properly, and the need of interdisciplinary communication and collaboration to timely assess the pressure ulcer risk and take necessary actions.
